Diabetes, particularly type 2, is a major public health concern in the UK, affecting around 4.9 million people. It significantly contributes to the burden of cardiovascular disease, kidney failure, retinopathy, and amputations.

Appt has excluded type 1 diabetes from this target as this cohort is typically managed in secondary care.

Annual care needs:

  1. Blood pressure
  2. HbA1c
  3. Lipid profile
  4. U&Es
  5. uACR
  6. Diabetic foot examination
  7. Annual diabetes review

Rationale:

Regular annual reviews are essential for managing blood glucose levels, monitoring complications, and ensuring optimal medication. These annual reviews should be informed with results from tests indicating glycaemic control (HbA1c), renal function (eGFR and uACR) and cardiovascular risk factors (BP, lipid profile). In this way, these reviews will address cardiovascular risk, foot health, alongside mental wellbeing, helping to improve patient outcomes and reduce the risk of severe complications.

Please note, there are additional regular care needs required for patients with type 2 diabetes, that are not within the scope of Appt’s care coordination service. For example, retinal screening which is conducted outside primary care.
