Introduction to care coordination targets

In time, Appt aims to cover all of the long-term conditions (LTCs) most frequency managed in primary care. Appt has started with some of the most prevalent conditions which have annual care requirements, where proactive and effective clinical care can make a significant impact on patient care and health outcomes.

For each target condition included within Appt’s care coordination service, the relevant clinical guidelines, primary care incentive schemes and clinical evidence for best practice are reviewed and considered.

It is important to note that Appt includes care needs for conditions that may not be directly mentioned or mandated in UK guidelines for the target condition, but may be considered for these patients on account of the potential clinical benefits of undertaking these tests and checks. As a practice, it is important that you decide what is best for your clinical practice and patient population. If there are care needs included within Appt’s care coordination service that you do not wish to offer your patient population, we can customise your targets accordingly – just get in touch.

Appt facilitates automation of the annual care requirements for these conditions, but many patients will need more frequent monitoring and input for individual and clinical reasons, for example more severe illness or medication changes. This interim monitoring remains the responsibility of the practice to schedule, and is not within the scope of the current version of Appt’s care coordination service.
