Welcome to our Care Coordination Service!

We’re excited to have you on-board. This guide will walk you through the key steps to set up your service, ensuring a smooth and efficient onboarding process.

Why is this process important?

Completing the onboarding steps will enable seamless integration with your system, allowing us to coordinate patient care effectively. By the end of this process, your system will be set up for:

  • GP Connect integration
  • EMIS coding configuration
  • Patient list upload
  • Clinician skill matrix setup

Step 1: Book your technical set-up meeting

This is the first and most important step. In this meeting, we will:

  1. Configure GP Connect session templates to enable access to live appointment availability.
  2. Enable an EMIS coding integration to ensure correct data mapping.
  3. Discuss how conditions and care needs are configured within the system.

What you need for this meeting:

         ✔️ Access to both EMIS and GP Connect
         ✔️ A team member familiar with your appointment setup
         ✔️ A structured patient list (see step 3 for details)

Step 2: Set up your clinician skill matrix

The clinician skill matrix ensures that patients are matched with the right members of your team based on their care needs.

How to set it up:

  1. Your account manager will send you through a template of a skills matrix.
  2. Prepare a list of clinicians and their care capabilities.
  3. Enter your team's details into the template - we will use this to match patients to clinicians based on their care needs
  4. Forward this to your account manager who will configure this for your practice.

Step 3: Prepare and upload your patient list

To allow our service to coordinate care effectively, we need a structured patient list.

How to prepare the list:

Format the file as a CSV with two columns:

  • Date of Birth (DD/MM/YYYY)
  • NHS Number

Please bring this list to your technical set-up meeting, where we will guide you through the upload process.

Once uploaded, we will process the list and send out the first batch of invitations the next day.

Step 4: Go live!

Once your technical set-up is complete and your patient list is uploaded, your service will be fully operational. We will provide ongoing support to ensure everything runs smoothly.
